• Child Information Record - Please read the instructions of this doc carefully & fill out the form thoroughly.

• Parent Notification of Licensing

• Health Appraisal - Acquired from a PCP during a well check. Make sure to fill out Section 1 & sign on pg 2. (You have 30 days from your child’s first day of school to submit.)

• If your child has an allergy you must submit an action plan (from a PCP) and a Medication Permission form

• Child/Family Profile
